When Is NY State of Health Open Enrollment for 2027? Dates, the December 15 Deadline, and What Changed

Quick answer NY State of Health open enrollment for 2027 coverage runs from November 1, 2026 through January 31, 2027. Pick a plan by December 15, 2026 and it starts January 1, 2027. Pick one after that and you are generally looking at February 1, with January uncovered. Two things are different this year. The state approved an average 6% increase for individual plans (insurers asked for 20.6%), and in Western New York the approvals range from 0% at Independent Health to 12.2% at Highmark and Univera. And the Essential Plan cutoff dropped to 200% of the poverty level on July 1, so a lot of people who never shopped the window before are shopping it now. The Essential Plan, Medicaid, and Child Health Plus have no window at all. Neither do the group plans I use for healthy self-employed New Yorkers. I educate, you decide.

What are the NY State of Health open enrollment dates for 2027?

Every row comes from NY State of Health's own documents. The two dates marked "expected" follow the pattern New York has used every year, including last year's official notice. The 2027 notice had not been posted when I wrote this. I will update the table the day it is.

Miss December 15 and you are most likely starting February 1. That is a full month with no coverage, right after the holidays, which is exactly when people find out they need it. Decide early. Then enjoy December.

How much are New York health insurance rates going up in 2027?

Less than the insurers wanted, and a lot less than the headlines from June suggested. On September 4, 2026 the New York State Department of Financial Services finished its review. Insurers asked for an average 20.6% on individual plans and got 6.0%. They asked for 23.7% on small group plans and got 8.0%. DFS puts the savings at about $1.6 billion across roughly 860,000 New Yorkers: $324 million for the 220,000 or so on individual plans and $1.25 billion for the 630,000 or so on small group plans.

Averages hide the story, though. In Western New York the approvals land all over the map:

Read that table like a detective. If you are on an Independent Health individual plan, your rate held flat and staying put may be right. If you are on a Highmark or Univera individual plan, a 12.2% average increase on top of last year's is the kind of letter that makes people say the math doesn't math. That letter is your cue to shop, not to sigh and pay it. I explain the "why" behind these letters in Why did my health insurance go up?

Sticker prices for one adult in New York, 2026 plan year: about $610 a month for the lowest-cost Bronze plan, $817 for the benchmark Silver, and $1,057 for the lowest-cost Gold, per KFF, before any subsidy. What else changed for 2027 on NY State of Health?

The window itselfUnchanged. New York sets its own dates. A federal rule that would have shortened marketplace windows for 2027 was vacated by a federal court on June 12, 2026 (City of Columbus v. Kennedy), and the appeal is set for argument October 30, 2026. Either way, December 15 is my advice.

Which New York programs have no open enrollment window?

Three of them, and they cover most of the state. NY State of Health says the Essential Plan, Medicaid, and Child Health Plus are open for enrollment year-round. Last year's notice put the total at over 6.7 million New Yorkers enrolled through the marketplace, and only about 220,000 of them were in Qualified Health Plans, the one product with a window.

What is the special enrollment period, and what counts?

Outside the window, a Qualified Health Plan takes a qualifying life event: losing other coverage, marriage, a baby or adoption, a permanent move, and a handful of others. NY State of Health gives you 60 days from the event to enroll (NY State of Health, Special Enrollment Periods). The most common event by far is a layoff, and it starts two clocks at once: 60 days to pick a marketplace plan and 60 days to elect COBRA. Use the first week, not the eighth. One trap: dropping a plan on purpose is not a qualifying event. Quitting COBRA early, or letting a plan lapse because you stopped paying, does not open a new window. Decide on purpose, not by default.

What should I actually do before November 1?

Here is the part the marketplace's own materials will never tell you, because it is not their job. The November to January calendar governs individual plans. It does not govern group coverage. And for a healthy New Yorker who earns too much for a subsidy, the group door is usually the one worth pricing first.

The pre-established ERISA group plan is the door I lead with. ERISA is the 1974 federal law that governs employer benefit plans, and it overrides New York's community rating. Group plans already exist under it, and a self-employed person, a 1099 contractor, or a small business's people can be merged into one. Group rates. A true PPO network, MagnaCare in New York and New Jersey or PHCS and MultiPlan nationwide, instead of the narrow regional networks most marketplace plans carry. A policy you own and take with you to the next job. The business can be the payer. No employees required, and one simple compliance step I walk you through. When that door does not fit, I customize the plan based on the situation. Often that is cost-effective, customized individual plans instead of a one-size-fits-all plan, layered with gap protection or accident coverage where the math says so. And sometimes the honest answer is a marketplace plan. I will tell you which, with the numbers next to each other.

Healthy, self-employed, income over the subsidy line

Price the pre-established ERISA group plan first, in any month. Then compare it against a marketplace plan at sticker. Your renewal letter's percentage is the tiebreaker. The New York doors, in order, are here.

Income under the subsidy lines

A subsidized plan or the Essential Plan may be your best deal, and I will tell you so. The trade-offs: narrower networks, an income estimate you have to get right, and repayment at tax time if you guess low. Run the subsidy calculator, preset for New York, then bring me the number.

You have no window to wait for. Three routes: traditional fully insured small group (8% average approved increase for 2027, 0% to 13.6% by carrier), the pre-established ERISA group merge, or no group plan at all. Guaranteed-issue coverage, for now. New York marketplace plans cannot ask health questions. Take that protection, do not miss the window, and we revisit the other doors later.

The open enrollment window only applies to Qualified Health Plans. Since July 1, 2026 the Essential Plan income limit is 200% of the federal poverty level, down from 250%, so people between 200% and 250% who lost it now shop the Qualified Health Plan window like everyone else.

What is the special enrollment period on NY State of Health?

Did the federal rule shorten New York's open enrollment for 2027?

No. A 2025 federal rule would have shortened the annual window starting with the 2027 plan year, but on June 12, 2026 a federal district court vacated that provision in City of Columbus v. Kennedy, and the government's appeal is scheduled for argument on October 30, 2026, per the Georgetown Health Care Litigation Tracker. New York runs its own marketplace and has published November 1, 2026 through January 31, 2027 for 2027 coverage. My advice stands either way: be done by December 15.

Do I have to wait for open enrollment to get health insurance in New York?

Only for an individual Qualified Health Plan. The Essential Plan, Medicaid, and Child Health Plus enroll year-round if you qualify. Group coverage is not tied to the individual calendar, and that includes the pre-established ERISA group plans I use for healthy self-employed New Yorkers: group rates, a true PPO network, and a policy you own, started in any month.
